Frequently asked questions

What is the difference between an incident and a near miss?

An incident is an event that caused injury, ill health or damage. A near miss is an event that could have caused harm but did not. Near misses are valuable because they show where the next incident is likely to happen, before anyone is hurt.

Will more reporting make our safety numbers look worse?

At first, often yes, and that is usually a good sign. When reporting becomes quick and easy, more near misses and hazards are reported. That gives your team more chances to fix problems before they cause injuries.

Can reports be made anonymously?

Your organization can choose how reporting works, including whether hazard and near-miss reports can be submitted without a name, so people feel safe to speak up.

How are investigations handled?

Each incident can be investigated with a structured root-cause analysis. Findings become corrective actions with owners and deadlines, and lessons learned can be shared with other sites.
